When should you use this form?

Use this form if you are unsure which JREB review pathway applies to your project, or if this is your first submission to JREB.

This intake form allows the JREB Secretariat to review a brief description of your proposed research or activity and advise you on:

• whether JREB review is required
• which review pathway applies (Exempt, Delegated, or Full Board)
• which documents will be required for a full submission

Important:

• This form is not an ethics application
• Submitting this form does not constitute ethics approval
• No documents are required at this stage
