Please read:

  • All applications must be started by the Principal Investigator. They can choose members of their team to collaborate on this submission (members will need to create their own accounts), however the application can ONLY be submitted by the PI. 
  • You can save a draft of your work if you would like to finish filling out the form at a later date. There is an autosave feature, but you can also manually save your draft by clicking the button at the bottom of the form.
  • We will follow up with the PI about the submission by email. Please be sure to safelist notification emails from Submittable and check the email you used to sign up for your Submittable Account regularly.

This application is for minimal-risk research involving the secondary use of existing data (e.g., retrospective chart reviews, database analyses, administrative data, or previously collected records).

This pathway is appropriate only when:

  • No new participant recruitment or contact will occur.
  • No intervention or interaction with participants will occur.
  • Data already exist at the time of submission.

If your study involves participant contact, prospective data collection, or intervention, please use the Research Ethics Application.

Use this project for all non-exempt human research requiring JREB review.

This application applies to studies that may qualify for Delegated Review or Full Board Review. 

Applicants do not select the review level. JREB assigns the appropriate review pathway based on the information provided in the application and risk-screening questions.

Some studies may require completion of an additional ethics and risk addendum (TAHSN) following submission.

Important:

  • Submission does not constitute ethics approval
  • Incomplete applications will be returned
  • JREB may request revisions or additional information before approval
  • If your study already has CTO approval, it does not require approval from the JREB. However, it must be registered to us through the CTO Notification Form.

Investigators submitting to JREB for the first time must complete the Pre-Submission Intake Form prior to submitting a Research Ethics Application.

This project is intended for Program Evaluation and Quality Improvement activities that meet JREB’s exemption criteria and do not constitute human subjects research requiring Delegated or Full Board review.

Submissions that involve research interventions, randomization, or increased risk to participants may be reclassified by JREB and routed to a different review pathway.
